Physical Education Medical Excuses

Doctor's notes excusing students from physical activity must be handed in directly to the nurse (not the P.E. teacher) during homeroom. Students will receive a blue note from the nurse that must be shown to the P.E. teacher, coaches and recess teacher, if applicable. An alternate assignment will be given to students who are excused from P.E.

If a student is able to participate on a limited basis, it must be explained in writing. The teacher will modify physical activity, if appropriate.

An excuse note extending three days may not be from a parent. Extended excuse notes must come from a doctor.

All doctor's and parent's notes must include:

  • the date
  • student's name
  • reason for the excuse from physical activity
  • return to activity date, or a new note will be required


Email Updates
Please make sure you update your email addresses in the parent portal. We frequently use email to communicate IMPORTANT messages and have recently discovered that quite a few are not working.

Looking forward to seeing you all soon, Please call the absentee line at 201-816-4910 by 8:30 AM if your child will be absent or late. Any student absent for 3 or more consecutive days will require a doctor's note in order to return to school.  

STUDENTS WITH INJURIES

To ensure the safety and well being of all our students, please advise the nurse of any injury that may alter or interfere with your child's functioning during the day. Any student returning to school on crutches, with braces, or with casts must report to the Health Office before the start of the day.

Student Health Services

As mandated by state law, all students will be screened for height, weight and blood pressure every year. If you do not want your child to receive these services, please notify the Health Office in writing by the 15th of September.

Medication in School

If your child requires routine or emergency medication during school hours, parental/guardian written permission and doctor's orders are required. For your convenience, the administration of Medication Form in the files below may be downloaded and returned to the Health Office.
